April is Advance Care Planning Month: Encouraging conversations about what matters most

April is Advance Care Planning Month, a time dedicated to encouraging individuals and families to think about what matters most to them and to share their wishes for future health and personal care.

Advance Care Planning (ACP) is the process of reflecting on personal values, beliefs, and preferences for care, and discussing those wishes with the people who are important to you. These conversations help ensure that your voice is heard and that your loved ones understand the kind of care you would want if you were unable to speak for yourself.

“Advance Care Planning gives people the opportunity to think about their goals, values, and what quality of life means to them,” says Home Hospice North Lanark. “By sharing these wishes with family and loved ones, individuals can help guide future health care decisions and provide peace of mind for everyone involved.”

Home Hospice North Lanark supports individuals and families in starting these important conversations. Through community workshops, trained volunteers, and online resources, the organization helps people learn about Advance Care Planning and how to communicate their wishes.

Residents are encouraged to take time this April to start the conversation with loved ones and learn more about the steps involved in creating an Advance Care Plan.
