
Perhaps you think that it is not for you because you have either considered and made your own arrangements for after death, or because you are hopefully a long way from having to consider the end of life with any concrete planning.
But that is not why Home Hospice North Lanark is offering once again a 1.5 hour presentation to adults in Carleton Place and Almonte, April 14 and 15. In fact, this presentation is about living not dying!
This Advance Care Planning presentation, developed by the province of Ontario, is directed to every adult citizen to make you aware of the legal implications of providing informed consent for health care procedures and decisions in the event you cannot speak for yourself, temporarily or longer. The presentation, supported by PowerPoint slides and a booklet for each participant is entirely free of charge.
It is presented by a trained volunteer who is a retired nurse, supported by a retired social worker and a local lawyer. You are free to ask questions and to clarify your understanding of concepts like informed consent, substitute decision maker, power of attorney, and how to address what is sometimes a difficult conversation with loved ones and other friends or family members.
By the end of the presentation, you will leave knowing who your substitute decision maker is, how to appoint a power of attorney for personal care, and some ideas about how to communicate your own beliefs and values about your health care decisions to those who most need to know in the event you are unable to speak for yourself. It is a gift to your family and peace of mind for yourself!
